Emergency Card Verification

The Emergency Card is an important document that parents/guardians complete to provide current information about their students. This information includes emergency contacts and details about transportation in case of an emergency, and is kept on file at your child's school. A separate Emergency Card must be completed for each student, and you will need your student's birthdate, student number, and 5-digit pin (provided by your school) to access the form.

Please watch this guide on how to complete the Emergency Card Verification Form. For more information or assistance, please contact your child's school.
