Student Seeking to Self-Enroll

Enrollment and proof of documentation must be done in consultation with the
Pupil Personnel Worker (PPW) for verification.

Additional Documents Needed

For a student to self-enroll, they should be living independently and have a separate residence from that of the parent/guardian.

Residency Documentation

Homeowners
Documents accepted by those who own or lease a residence include a mortgage document issued within 60 calendar days, deed, valid rental agreement, military housing lease and one(1) of the items listed below:

  • Utility bill/cable bill or work order issued by the utility or cable company

  • Bank statement

  • Pay stub

  • W-2 Form or Form 1099 issued the previous year

  • Valid Commercial Driver’s License

  • Any government-issued documentation, such as Social Security check, child support order, Department of Social Services food or community medical assistance letter, or unemployment award

  • PPW verification form verifying residence after a home visit

Renters
Documents accepted by those who live in a residence owned or leased by an individual or family include a notarized Tenant Residence Verification Form (TRV)  (Formulario de verificación de residencia del inquilino) with three(3) of the following additional items:

  1. Copy of the homeowner/landlord's deed, mortgage document, or rental agreement

  2. Utility bill or cable bill from the homeowner/landlord; and,

  3. Two items from the list below in the name of the parent/guardian enrolling the student:

  • Utility bill/cable bill or work order issued by the utility or cable company

  • Bank statement

  • Pay stub

  • W-2 Form or Form 1099 issued the previous year

  • Valid Commercial Driver’s License

  • Any government issued documentation, such as Social Security check, child support order, Department of Social Services food or community medical assistance letter, or unemployment award

  • PPW verification form verifying residence after a home visit

Evidence of Birth

One or more of the following:

  • Birth certificate (preferred)

  • Passport/Visa

  • Physician’s certificate

  • Birth registration

  • Baptism certificate

  • Church certificate

  • Hospital certificate

  • Parent affidavit

Medical Records

All three(3) required.

  1. Immunization (shot) record (see Vaccination Requirements for Children Enrolled in Preschool Programs and in Schools - State of MD)

  2. Lead certificate for students entering Prek, K, or first grade

  3. Record of Physical Examination for students entering a Maryland public school for the first time. (within nine(9) months prior to entering or six(6) months after entry)

Documents Needed Only if Applicable

  • Court ordered child custody document (if one exists)

  • Maryland Transfer Form SR7 if transferring from a Maryland Public School

  • Copy of last report card/high school transcript

  • Documentation of Special Services

    • Special Education – current IEP, evaluations

    • 504 plan

    • ESOL services

    • Gifted/Talented
