If you would like your student to take advantage of this opportunity, please complete the attached referral form. Since spots are limited, we encourage you to fill out the form as soon as possible to secure a spot for your student. Once the form is completed, we will follow up with you to schedule an appointment time.
In addition to providing services for students, America’s Best Contact and Eyeglasses is also offering free eye exams and glasses for adults.
